Microneedling is based on a simple biological principle: when you create small, controlled injuries in the skin, the body's wound healing response rebuilds the damaged tissue with new collagen and elastin. The key word is controlled. The injuries must be deep enough to trigger dermal remodeling, but shallow enough to heal quickly without scarring.

Fibroblasts (the cells responsible for producing structural proteins) are activated by the growth factor signals from Phase 1. The micro-channels close within hours, but the remodeling process continues for weeks beneath the surface.
The initial Type III collagen is gradually replaced by organized Type I collagen. The new collagen fibres align and cross-link, increasing the tensile strength and structural integrity of the skin. This is the phase where visible improvements in texture, firmness, and scar appearance accumulate.
This is why microneedling results improve over months. You are not seeing the full benefit at week 2. The collagen remodeling process continues for 3 to 6 months after a single treatment.

The timing is critical. Growth factors applied to intact skin sit on the surface. Growth factors applied through open micro-channels reach the fibroblasts and stem cells that drive remodeling. This is why the combination of microneedling plus growth factors produces results that neither treatment achieves alone.
Acne scars (particularly atrophic scars like ice pick, boxcar, and rolling scars) are caused by collagen loss or abnormal collagen formation during the healing of inflammatory acne. Microneedling triggers new collagen production beneath the scar, gradually filling in the depression and improving texture. Multiple sessions (typically 3-6, spaced 4-6 weeks apart) produce cumulative improvement.
Fine lines form when the dermal collagen and elastin network thins and fragments with age and UV exposure. The new collagen produced in response to microneedling thickens and strengthens this network, reducing the appearance of fine lines. This is structural improvement, not temporary hydration plumping.
Pore size is partly determined by the collagen support structure around each pore opening. As collagen degrades, pores lose their scaffolding and appear larger. New collagen production around pore openings tightens this scaffolding.

Microneedling can improve hyperpigmentation by accelerating the shedding of pigmented epidermal cells.
